15-259 Coil Balance Pivot Shoe

I believe I need a new pivot shoe for my window. The window information is in one of the photos. The current shoe is 1.5 inches tall x 1.25 inches wide x 0.875 inches thick.

Please identify the pivot lock shoe mechanism for this full tilt window. I need to order 4. Thanks, John

The balance is for the upper sash in a double hung vinyl frame window. The glass is double pane, about 38" x 20". See the above name tag attached to the frame. Looking to ID this tilt pivot shoe. Many of them have broken in my windows and need to be replaced. I believe the windows were bought from home depot in the past.
