Can you help me find this casement operator 30146

The crank mechanism of one of my casement windows doesn't work properly anymore. When turning the crank, it feels as if the gear is missing some of its teeth. Hello, I am having trouble finding a suitable window operator replacement for the attached photos. This window was forced open from a closed position and it ripped the metal from the channel. There is a plastic slider piece that is supposed to be in that track that I need to replace as well.

The crank gear mechanism I need is from the Crestline casement windows that were in stalled in my house about 43 years ago. The overall length of the arm and gears is about 12 inches. Reverse side TRUTH P/N is LH45135
